As part of our efforts to deliver the best possible care to patients, Wells Community Hospital Trust is running a patient survey which we'd ask anyone who has been seen or treated at the hospital to complete. Results will be collated every six months and made available on this website and, in paper form, at the hospital itself.

If you've been examined or treated at Wells Community Hospital, please help us to improve our services by completing the survey. You can download the survey in PDF or Word format using the links below or fill it in on-line. Alternatively, you can collect a printed survey form from the hospital reception.
