Patient Survey

As part of our efforts to deliver the best possible care to patients, Wells Community Hospital is running a patient survey which we'd ask anyone who has been seen or treated at the hospital to complete. Results will be collated every six months and made available on this website and, in paper form, at the hospital itself.

Completing the survey

If you've been examined or treated at Wells Community Hospital, please help us to improve our services by completing the survey. You can download the survey in PDF or Word format using the link below or fill it in on-line. Alternatively, you can collect a printed survey form from the hospital reception.

For translation into other languages or any other help with the survey, please ask at reception or contact us by e-mail.

If you have any other feedback or concerns about your visit to the hospital, please call 01328 711996 and ask to speak to the Senior Nurse. Alternatively, you can call ICAS (the Independent Complaints Advisory Service) on 0845 456 1084 or the Health Care Commission on 020 7448 9200. All information will be treated with the strictest confidentiality.

VISION STATEMENT
The vision is for local people in North Norfolk, served by Wells Hospital for so many years, to take the responsibility for developing and managing a new, expanded hospital and community health service, appropriate to their present and future needs, and with the highest possible standards of care for the individual. Our purpose is to make sure that anyone experiencing illness or surgery shall receive the very best rehabilitation to rapidly return home in good health.
